We request and appreciate payment at the time of service for the portion of the fee for which you are responsible (e.g. co-payments, deductibles, and past-due account balances.) Statements are mailed monthly. We accept most major credit cards for your convenience.
Our staff will make every effort to simplify all billing insurance claim procedures. We participate with many insurance plans; however, we recommend you call your insurance company to verify that we participate with your plan. Please bring your insurance card with you to every visit so that we may ensure that our records are accurate. If you do not have insurance coverage, full payment is required at the time of service.
We will be happy to assist you with a referral to a provider that your primary care physician has recommended. If a referral is necessary, we request notification at least 72 hours in advance of your appointment. Same day referrals will be processed for urgent appointments.
